Overview
This form is to be completed only after a student has attempted to resolve a grade dispute informally with the course instructor and, if unresolved, with the department chair or program director. Do not submit this form to the Office of the Provost. Completed forms are submitted to the Division Dean of the division that houses the course department under dispute. Identify the appropriate Division Dean using the reference list below, and contact that Division Dean's office for submission instructions.
